What is a Care Manager?
A Care Manager is a geriatric professional that can help older adults and families develop and implement a plan of care that will meet their long-term needs. They offer customized consultations and advice to their clients, helping them make decisions that are appropriate for them.
Geriatric care management consists of a wide range of services and interventions that address every aspect of aging. A comprehensive assessment initiates the process and evaluates the client’s medical, social, emotional and financial well-being as well as their self-care abilities, safety needs and family relationships. The assessment determines the types of services and level of care that may be needed and matches the need to the appropriate resources through an individualized plan. Services may be as fundamental as overseeing household operations or as complex as arranging for the transition to residential placement. Care managers coordinate services, provide ongoing monitoring and maintain communication with family members. Care Managers can also negotiate family dynamics and differences, helping to sort out all the various problems and concerns. The care manager often must be creative and find non-medical solutions. By working closely with clients, caregivers, families, and friends they can build a strong relationship that provides the caring, loving support network seniors need to continue to remain at home.
Your care manager can advocate for you, liaison with family and/or medical professionals for you, and provide written and verbal updates to family that are not close by.
