What is a Geriatric Care Manager?

A geriatric care manager is professionally trained to help older adults and families develop and implement a plan of care that will meet their short-term or long-term needs.

Geriatric care management consists of a range of services and interventions that address every aspect of aging.  A comprehensive assessment determines the types of services and level of care that may be needed and matches the need to the appropriate resources in the community.

Services may be as fundamental as overseeing household operations or as complex as arranging for the transition to residential placement.

Care managers coordinate the services, provide ongoing monitoring and maintain communication with family members who are not close by.

Geriatric care managers offer customized consultations and advice to their clients, and help them make decisions that are appropriate for them.

Comprehensive Client Assessment and Evaluation

We start with a comprehensive evaluation of the client’s medical, social, emotional and financial well-being as well as their self-care abilities, safety needs and family relationships.

The assessment determines the types of services and level of care that may be needed and matches the need to the appropriate resources through an individualized plan.
